Dog Boarding & Sitting Agreement & Liability Waiver.......

 For information only.  You will be required to sign prior to boarding a pet or our acceptance to providing you pet-service.

I hereby acknowledge that I have voluntarily requested and reserved to have my canine boarded at a Paws Plus You facility or location and/or reserved pet sitting services with members of Paws Plus You LLC.

This Pet Care Agreement is made as of this ____________ day of _______________ 20______, by and among/between:

The care and services will be provided at the following address:

Time and Payment.  Pet Sitter shall care for Pet Owner’s pet(s) as identified and described in the Pet Care Information Form for the time period beginning _________________________, 20______ until ________________________, 20_____.  In consideration for Pet Sitter’s care and services, Pet Owner shall pay Pet Sitter _________ per day (day equals past 1pm day of departure).  The payment is due to Pet Sitter at the completion of the care and services and/or at the time of pick up.

Emergencies:  If there is an emergency, Pet Sitter will make all reasonable efforts to first contact Pet Owner.  If Pet Owner is unable to be contacted, Pet Sitter shall reach out to Pet Owners listed emergency contact(s). 

Pet Owner authorizes pet sitter to seek and obtain medical attention and treatment as deemed necessary by a veterinarian.  Pet Owner agrees to reimburse Pet Sitter for reasonable expenses incurred arising from/relating to:  medical care and treatment, emergency medical care, and services provided by a veterinarian. 

Injury and Indemnification.If pet accidentally dies, becomes ill, injured, or lost, Pet Owner waives and releases Pet Sitter from any claim arising from such an incident.  If the pet(s) bites or injures Pet Sitter, any other human or animal, Pet Owner will be responsible for any resulting injury.  In addition, Pet Owner shall hold harmless and indemnify Pet Sitter against any and all costs, expenses, losses, liabilities and claims arising out of or relating to any acts of the pet(s) except if arising out of the negligence or intentional misconduct on the part of the Pet Sitter.

Termination of Agreement.  Paws Plus You LLC reserves the right to terminate this contract at any time before or during its term if Paws Plus You LLC, in its sole discretion, determines that Pet Owner’s pet poses a danger to the health or safety of the Pet Sitter.  If concerns prohibit Pet Sitter from caring for pet, Pet Owner authorizes pet to be placed in a kennel, with all expenses to be charged to client.

Governing Law.  The terms of this Agreement shall be governed by and construed in accordance with the laws of the State of Michigan, not including its conflicts of law provisions.

Severability.  If any provision of this Agreement is held to be invalid or unenforceable in whole or in part, the remaining provisions shall not be affected and shall continue to be valid and enforceable as though the invalid or unenforceable parts had not been included in this Agreement.

Entire Agreement and Amendment.  This Agreement contains the entire understanding between the Parties and supersedes and cancels all prior agreements of the Parties.  This Agreement may be amended or modified only by a written agreement signed by all the Parties.

I hereby acknowledge that I have voluntarily applied to participate in dog services with Paws Plus You LLC.

Dog Outings.I am aware that there are inherent risks and hazards involved in activities with and around dogs, and I am voluntarily participating in these activities with knowledge of potential dangers. I am aware that any dog, regardless of training, handling, or environmental circumstance, is capable of biting and I expressly acknowledge the risks therein.

Training Classes.In order to participate in dog training classes or other activities, I, being fully informed of such risks and hazards, agree to assume all risks of such occurrences. I hereby waive any and all claims or actions that I or my guardians or representatives may have, from any and all personal injury to myself, my dog, children in my charge, or harm to property or person caused directly or indirectly, through action or inaction of self or others, by acts that might occur in dog training classes, any other format of training activities or secondary training without trainer present or engaged.

I AGREE TO INDEMNIFY PAWS PLUS YOU LLC AND ITS EMPLOYEES AND AFFILIATES FROM ANY AND ALL CLAIMS BY MYSELF, MEMBER OF FAMILY, OR ANY AGENT WHILE WITHIN BOARDING AND TRAINING FACILITIES, WITHIN MY HOME PROPERTY, OR IN THE GENERAL PUBLIC AS A RESULT OF ANY ACTION OR INACTION, OF EITHER MY DOG OR ANY ANOTHER. I ALSO AGREE TO ASSUME SOLE RESPONSIBILITY FOR INJURY OR DAMAGE CAUSED BY MYSELF, CHILDREN IN MY CHARGE, OR BY THE DOG I OWN OR HANDLE AND FURTHER AGREE TO INDEMNIFY, DEFEND AND HOLD THE PET SITTERS, TRAINERS, ASSISTANTS AND PROPERTY HARMLESS FROM ANY DAMAGE, LOSS, LIABILITY OR EXPENSE, INCLUDING LEGAL COST AND ATTORNEY'S FEES, WHICH RESULT FROM DAMAGE CAUSED BY MYSELF, CHILDREN IN MY CHARGE, OR BY THE DOG I OWN OR HANDLE.

PAWS PLUS YOU IS AN INDEPENDENT BUSINESS AND IS NOT IN ANY WAY AFFILIATED WITH SUTTON PARTNERS, LLC, VISCO HOLDINGS, LLC, OR THE LODGE FOR PAMPERED PETS AND HOLDING SUTTON PARTNERS, LLC, VISCO HOLDINGS, LLC, AND THE LODGE FOR PAMPERED PETS HARMLESS FOR ANY INJURIES OR DAMAGES SUFFERED WHILE ON THE PREMISES.

I have provided current vaccination documents from a licensed veterinarian and a negative fecal report.

Paws Plus You LLC may use pictures ofmy dog for publicity or promotional purposes of Paws Plus You LLC without liability or obligation of any kind to me; however, no information or pictures can be sold or shared otherwise without additional consent.
